Consent Form
Understanding Attitudes and Predictive Ability
To project participant:
This is a research project conducted by Catherine Veisel, a graduate student in the Psychology Department at California State University, Los Angeles and Dr. David J. Weiss, faculty advisor. In this study we hope to learn more about the opinions of college students on certain social situations and, in some cases, determine individuals' ability to predict what happens next. Some of the issues discussed will be of a somewhat sensitive nature. You must be at least 18 years of age to participate in the study.
Please keep your answers private. We appreciate your cooperation in being completely honest, and, since this is totally anonymous, your name will never be associated with your answers. In this way, no one will ever know how you responded.
You are under no obligation to participate in the study. You may refrain from answering any questions that make you uncomfortable and you may choose to withdraw from the study at any time without penalty. If you choose to take part, I will distribute a questionnaire after you have signed this consent form. Consent forms are standard procedure in psychological research, and, to preserve anonymity, the signed forms are collected before you fill out the questionnaire. That will prevent any chance of the signed form containing your name being paired with your questionnaire, which has no identification.
If you have any questions or concerns regarding the study, you may contact Catherine Veisel at (818) xxx-xxxx or Dr. David J. Weiss at (323) 343-2271.
By signing this consent form, you indicate that you have read the form and agree voluntarily to participate in the study.
I agree to participate in the Understanding Attitudes and Predictive Ability Study, as set forth above.
